Step-1: Register/Schedule an appointment
To register and schedule an appointment you can either

1. Call us at 919-439-6120, or
2. Request an appointment online and we will contact you to schedule based on your preferences. Appointment Online
   

Step-2: Submit new patient forms
We need the completed patient forms on your first visit. After completing the form, please print and sign the treatment agreement and past medical record request forms. We will not be able to receive your medical records without your signature.

• Online Patient Registration Submission OR manually, print and submit: Alpha New Patient Registration
  
• Online Release of Records Request OR manually, print and submit: Alpha Release of Records Request
  

If you completed the forms manually (printed and filled), then submit in one of the these ways:

1. Fax them at 919-246-4420
2. Bring them along on your first visit
3. Upload completed forms online
   
4. Come 15 minutes prior to the appointment and fill them out
